How to Apply
Interested candidates can download the application form from the website: http://pgimer.nic.in/. Thereafter, the duly filled downloaded application form must be sent with the requisite fee by DD in the name of Director, PGI. One can get the application form and information brochure in person from the office of Registrar, against a payment of Rs 500 for General/ OBC/ OPH candidates and Rs 400 for SC / ST candidates at the Counter (Kairon Block, Room No 309) form 10.30 am to 12.30 pm & from 2.30 pm to 3.30 pm on all working days except Saturdays (on Saturdays forms will be available from 10.00 to 12.30 Noon). Application form and prospectus can also be obtained by post from the Office of Registrar by post by sending a request for the application form along with a self addressed thick envelope of size 24 x 15 cm bearing postage stamps of Rs 30 and bank draft preferably from any SBI branch payable at SBI Medical Institute Branch (code no. 1524) Chandigarh for General/OBC/OPH Rs 500 and for SC/ST Rs 400 (indicating category) drawn in favor of the Director, PGI, Chandigarh. However, prospectus for DM / M Ch will be available for Rs 500 only as there is no reservation for the same.
