Interested candidates can download the application form from the institute’s website: http://www.sdmimd.ac.in/. Thereafter, the duly filled application form must be submitted along with a DD of Rs 1350 drawn in favor of SDM Institute of Management Development payable at Mysore. Application forms are also available at Various Coaching Centers or selected branches of HDFC Bank (detailed list of address of the Coaching centers and HDFC Bank Branches are available at the aforementioned website) against a cash payment of Rs 1350. The same can be obtained in person from the SDM Institute of Management Development, Site No 1, Chamundi Hill Road Siddarthanagar, Post Mysore 570 011 against a cash payment of Rs 1350. All the completely filled application form along with DD / Photocopy of Challan / Photocopy of Coaching Centers receipt must be send addressed to the Office of SDM Institute of Management Development (SDMIMD), Mysore on or before the last date mentioned.
