Interested candidates can obtain the application form and prospectus in person from the following centers - i) Office of the Principal, Govt. College of Nursing, Raj Bhavan Road, Somajiguda, Hyderabad ii) Office in-charge, Dr. NTR UHS, Camp Office, Koti, Hyderabad or iii) Registrar Dr. NTR University of Health Sciences, Vijayawada by paying Rs 600 for General Class and Rs 450 for SC/ST category candidates in the form of a DD on any nationalized Bank 2009 in favour of the Registrar Dr NTR University of Health Sciences, payable at Vijayawada. One can also obtain the application form and prospectus by post form the Office of the Registrar Dr. NTR University of Health Sciences, Vijayawada – 520 008, accompanied with a DD of Rs 650 for General Class and Rs 500 for SC/ST categories drawn not earlier than July 4, 2009 on any nationalized bank in favour of Registrar, Dr. NTR University of Health Sciences, Vijayawada. Thereafter, the duly filled application form must be submitted to the Registrar, Dr NTR University of Health Sciences, Vijayawada, on or before the last date mentioned.
