How to Apply
Interested candidates can download the application form and information brochure form the website: http://www.sgpgi.ac.in available from April 24, 2009 to May 20, 2009. Applicants will have to fill the application form and submit it online. One must take out a print out of the submitted application form and send it to the Executive Registrar SGPGIMS Lucknow 226014. The application form must be accompanied with a bank draft of Rs 1000 towards examination fee drawn in favor of Director, SGPGIMS, Lucknow Academic Account, drawn on SBI, SGPGI Branch, (Code No. 7789), payable at Lucknow and also with the essential documents in support of age, qualification, experience, reservation etc and duly signed by the candidate, either through courier or speed post in a envelope super scribed “Application for entrance exam of SR (HS)/DM/MCh/PDCC/PDF (with specialty).
