How to Apply
Interested candidates can download the application form from University’s official website http://www.bfuhs.ac.in. Thereafter, the duly filled application form should be submitted along with the list of essential enclosures given below and a DD of Rs 3000 in the favor of the Registrar, BFUHS payable at Faridkot. For further details about the admission and application procedure applicants are requested to visit the aforementioned website.
List of Essential Enclosures (Attested) For MSc Course
- Degree/Certificate from the Head of the institute where from passed BSc (Anatomy, Physiology, Biochemistry)/BDS.
- Detail Marks Card of BSc (APB)/BDS of all years.
- Character certificate from institution last attended.
- Certificate in support of claim under reserved category.
- Punjab residence certificate.
- DD of Rs 3000 (in original).
- Self undertaking (not availed any residence benefit in any other state).
- Self undertaking of gap year (if any).
- Matric or equivalent certificate for Date of Birth.
List of Essential Enclosures (Attested) For MPT Course
- All detailed marks certificates of qualifying examination i.e. BPT examination.
- Internship completion certificate
- Degree of BPT or equivalent certificate
- Certificate that BPT examination has been passed as a regular student from a recognized institution/board/university in the state of Punjab or other state.
- Board/Exemption certificate under clause 7 (a) to (e) of Punjab government
- Notification, in case BPT passed from outside Punjab.
- Matriculation certificate showing Date of Birth
- Character/Conduct certificate from the institution last attended
- Punjab residence certificate
- Certificate of reserved category from the competent authority, if any reservation is claimed
- Gap in studies affidavit, if applicable
- Migration certificate, if passed BPT or equivalent from board/university other than BFUHS, Faridkot
- DD in original
