The duly filled online application form along with proof of payment should be submitted to the Professor Academic, JIPMER. Candidates can obtain the application form from the counter of State Bank of India (SBI), JIPMER campus only. One can obtain the same through post by sending a request along with a DD or original challan of account transfer and with two stickers with candidate's address to the Professor Academic, JIPMER.
