Interested candidates can download the application form from the institute's official website: http://www.sdmimd.ac.in/. Thereafter, the duly filled application form must be submitted along with a DD of Rs 1350 drawn in favor of SDM Institute for Management Development payable at Mysore. Candidates can obtain the application form directly from the institution by the paying the cash on site. Application forms are also available at various coaching centers or selected branches of HDFC Bank (list available at: http://www.sdmimd.ac.in/how-to-apply). Candidates can obtain the application form from these centers by paying Rs 1350. Candidates should submit the filled in application form with DD/photocopy of Challan/photocopy of Coaching Centre's receipt (excludes TIME coaching centre) to the Admission Office, SDM Institute for Management Development (SDMIMD), No. 1, Chamundi Hill Road, Siddarthanagar Post, Mysore - 570011.
