How to Apply
Interested candidates may obtain the prospectus containing eligibility conditions, domicile criteria, application form and OMR form from HDFC Bank, Near D-Park, Model Town, Rohtak against a cash payment of Rs 2000 (Rs 500 for SC/BC of Haryana). One can get the prospectus containing application form through post by sending request for prospectus through Registered Post. The request must be sent to the office of the Controller of Finance, Pt B D Sharma University of Health Science, Rohtak on or before May 9, 2011 by sending a DD of Rs 2050 (Rs 550 for SC/BC of Haryana) drawn in favour of Controller of Finance, Pt B D Sharma University of Health Sciences, Rohtak payable at State Bank of India, Pt B D Sharma PGIMS, Rohtak (Code 4735) failing which the request will not be entertained. The duly filled and completed application form must be sent to the controller of Examinations, Pt B D Sharma University of Health Sciences, Rohtak latest by May 18, 2011 (up to 5.00 pm).
