The application fee is Rs 300 which is to be paid in the form of a non-refundable bank draft drawn in favour of “Director, SGPGIMS (Academic A/c), Lucknow” No. payable at State Bank of India, SGPGIMS Branch, Lucknow. The certified copies of all the testimonial is also needed to be attached along with the form. The duly filled in application form along with bank draft and enclosure documents should be sent to Director, Sanjay Gandhi Postgraduate Institute of Medical Sciences, Raebareli Road, Lucknow-226014 (U.P.)
