How to Apply
Interested candidates may obtain the application form from MPPEB Bhopal counter and designated institutions (list is given on the MPPEB official website: http://vyapam.nic.in/) from January 18, 2012 onwards on cash payment of Rs 1500 (Rs 750 for SC/ST candidates). The duly filled in application OMR form must reach Madhya Pradesh Professional Education Board (MPPEB) Office, Chayan Bhawan, Bhopal, Madhya Pradesh, latest by February 10, 2012 (up to 5.30 pm) through registered post / speed post. In addition, one can submit the application form in person at the abovementioned address. In-service candidates shall have to submit their In-Service form in the prescribed proforma as given in MPPEB rule book, along with the application form, to the Director, Health Services, M.P, 4th floor, Satpura Bhawan, Bhopal.
