UP Unaided Medical Colleges Welfare Association (UPUMCWA), Bareilly, Uttar Pradesh, has invited applications from eligible candidates for UP Combined Medical Entrance Test 2012, also known as UPCMET. UPCMET is an entrance test being conducted by UPUMCWA for admission into five year Bachelor of Medicine, Bachelor of Surgery (MBBS) programme, to be offered at the below mentioned unaided medical colleges of Uttar Pradesh State. Selection will be made on the basis of candidates’ performance in the entrance test, followed by counseling.

How to Apply
Interested candidates may obtain the application form by following the below mentioned modes:
- Online: Applicants may download or fill up the application form from the official website of UPUMCWA: http://www.upumcwa.org/. One has to enter all the details carefully as there will be no option of correction once the form is submitted. After the submission, candidates will get Confirmation Page which should be submitted along with an application fee of Rs 1000, paid in the form of Demand Draft, drawn on any nationalized bank in favor of UPUMCWA, payable at Bareilly.
- By Post: Candidates may obtain the information brochure and OMR application form by post on sending a requisition along with a Demand Draft of Rs 1050 (an additional Rs 50 for postal charges) and two self addressed stickers to: The Coordinator UPCMET-2012, First Floor, Anand Ashram Complex, Rampur Garden, Bareilly, Uttar Pradesh - 234 001. The DD should be drawn on any nationalized bank in favor of UPUMCWA, payable at Bareilly.
- In Person: Information brochure and OMR application form can also be obtained in person from selected branches of Oriental Bank of Commerce (list given on the website of UPUMCWA) or from the counters of the participating member colleges on cash payment of Rs. 1000.
The duly filled-in application form in the form of OMR / Printout / Confirmation Page (whichever is applicable) along with all enclosures (list given on the website) and DD of prescribed amount should be sent to: The Coordinator UPCMET-2012, First Floor, Anand Ashram Complex, Rampur Garden, Bareilly, Uttar Pradesh - 234 001 on or before the below mentioned last date of submission of application form. Note:
- Admit Card will be mailed to the candidates by post on their correspondence address as given in the application form.
- In case the candidates do not receive the admit card one week before the scheduled date of examination, they must visit the official website of association or contact 9897600270 / Coordinator at 9412761020.
- Duplicate Admit Card will be issued one day before the entrance examination from 10.00 am to 3.00 pm at the opted examination centre.
