Interested candidates may obtain the prospectus containing Eligibility conditions, Domicile criteria and Application form etc., from HDFC Bank, D. Park, Model Town, Rohtak (Haryana) or from the Controller of Finance, Pt. B.D. Sharma University of Health Sciences, Rohtak, on cash payment of Rs 2000 (inclusive of Examination Fee) at the counter. One can obtain the same through Registered Post by sending requisition letter (indicating prospectus for Entrance Examination of MD / MS / PG Diploma, MDS & M Ch / DM Courses for the session 2012) along with a Bank Draft for Rs 2050, drawn in favor of “Controller of Finance, Pt. B.D. Sharma University of Health Sciences, Rohtak” payable at State Bank of India, Rohtak. The duly filled in and completed application form along with requisite documents (details mentioned in the prospectus) should be sent to the Controller of Examinations, Pt B D Sharma University of Health Sciences, Rohtak, latest by February 10, 2012 (up to 5.00 pm).
