How to Apply
Interested candidates may obtain the application form and information leaflets from Institute of Medical Sciences (IMS), BHU, sales counter from March 19, 2012 onwards. The same can be obtained through post by sending requisition along with a MICR DD of Rs 100 along with a self addressed envelope of 25 x 16 cm and a request letter addressed to The Director, IMS, BHU, Varanasi – 221 005 mentioning the appropriate course and full address with contact telephone number. The envelope must be super-scribed as “REQUEST FOR APPLICATION FORM for DM / M Ch AUGUST 2012.”
In addition, application form and information leaflet can also be downloaded from BHU website: http://www.bhu.ac.in/ or http://www.imsbhu.nic.in/. The application fee for DM / M Ch course (Residency / Sponsored) is Rs 2000 which is to be paid through MICR DD drawn in favour of “Director, IMS, BHU” payable at Varanasi. The duly filled in and completed application form along with prescribed DD and all enclosures should be sent to the office of the Director, IMS, BHU latest by May 7, 2012.
