The application fee is Rs 1000 which is to be paid in the form of DD drawn in favour of Registrar, Baba Farid University of Health Science, payable at Faridkot. The duly filled in application form along with DD and requisite enclosure documents (list given in the website) should be sent to the office of Registrar, Baba Farid University of Health Sciences, Faridkot, latest by September 16, 2011.
